Carebridge is pleased to announce that we’re now utilizing the CAQH Application to simplify the process of maintaining your provider information, making it easier and more efficient for you. Best of all, it’s completely free! If you’re not yet a CAQH member, please complete the application to remain on our panel.
If you’re already a CAQH member, please be sure to grant Carebridge access to your records and re-attest every 120 days. For more details about CAQH, click CAQH.org

Need to let us know about changes to your practice or availability that are not on the CAQH site? Submit the online Provider Change Form and we will update our records.

Billing and Payment made easy
Submitting your billing is quick and easy through our online system.
Online Billing Form
Your billing forms will be processed with payments conveniently mailed out within 30 days. To ensure timely payments, please submit your billing forms within 45 days of the most recent session date. As a reminder, we offer interim billing to help you receive payments even faster! For questions or help, email the billing department, providers@carebridge.com, or call 866.993.8477.
Carebridge EAP is committed to sustainability and reducing our environmental impact, so we prefer electronic billing whenever possible. However, for your convenience, we've included a downloadable version of the paper bill should you need it. Information on how to submit paper billing is on the form.
